Starting from 0:00 December 1, 2020 ICT Time, all foreign passengers who are to fly to China from Cambodia should apply for Health Declaration Form via https://hrhk.cs.mfa.gov.cn/H5/. On this website, foreign passengers should declare their health status and upload 1) negative nucleic acid test and IgM anti-body test results and certificates (both tests must be conducted within 48 hours before boarding the flight); 2) passport information page; and 3) itinerary of air travel and other necessary information asked on the web page. The Chinese Embassy in Cambodia will examine and verify the above documents online, and then issue the Green Health Declaration Code with the "HDC" Mark to qualified passengers. Passengers must present the Green Health Declaration Code with the "HDC" Mark within its validity period to the airlines flying to China before boarding.

The specific requirements are as follows:

1.Make sure the nucleic acid and IgM anti-body tests be taken within 48 hours before flight departure time. The validity period of the certificate will be calculated based on sampling date. For example, if the flight departs on December 1, the tests must be taken no earlier than November 29.

2.Make sure the nucleic acid and IgM anti-body tests be taken at the institution designated by the Chinese Embassy in Cambodia (http://kh.china-embassy.org/eng/lsfws_1/t1801597.htm) .

3Since there are more than 20 direct flights from Cambodia to China every week, passengers are advised to take direct flights to China. Please note that the Chinese Embassy in Cambodia accept neither Health Declaration Form Application from transit passengers nor applications for certified Health Declaration Forms via email. All applications from direct-flight foreign passengers for certified Health Declaration Forms should be submitted online via https://hrhk.cs.mfa.gov.cn/H5/ .

4. It is known to the Embassy that National Institute of Public Health(NIPH) opens for nucleic acid and IgM anti-body tests everyday without exception.

5.Make sure the name, gender, date of birth and the number of the passport or other travel document as shown in the certificates are consistent with those shown on the passenger's passport or travel document. Passengers are requested to submit their applications for the Green Health Declaration Code as soon as they get their test results.

6.The requirements apply to passengers of all ages including infants.
